Please click on the link below (Application) you may also download, print and mail, email or fax your application. If faxing please send to: 706-356-0226

If mailing, please send to: New Beginnings Ministry 1258 Greater Hope Road Martin, GA 30557 and include a check or money order for the $25.00, non-refundable application fee.
If emailing or faxing the application fee can be made over the phone with a credit card or sent by mail.

Once your application is received, the application will be processed and arrangements for a phone interview will be made.

When filling out the form please complete all fields to the best of your ability. If you agree to it, please sign it and then proceed to the Application, Financial Statements and Questionnaire forms leaving NO blanks. Be sure to include a contact phone number where we may call or text you as well as an email if you’d like. Please use ONLY BLACK INK on printed application

If you are incarcerated, provide the number for the person who will arrange the phone interview.  A collect call will be accepted by New Beginnings as long as it has been set up between the contact person and New Beginnings Ministry.

You must be at least 18 years of age to apply

Program Details & Fees

PLEASE BE MINDFUL THAT FEES ARE NOT LIMITED TO ONLY $700.00 / MONTH BUT BASED ON A SLIDING SCALE FROM $700.00-$1,700 A MONTH PLUS SPENDING.

Please understand we require you to commit to 12 months minimum.

Please note:
No visits or phone calls for the resident’s first 30 days
(client will be able to write and receive mail during this time)
NO cell phones allowed in the visitation area
